Mission San Jose Baptism Requirements & Checklist
(Infant to 6 years of Age)Parent Information – Please enter a checkmark in spaces below indicating that you have the information needed for registration:

___ If you don’t live within San Jose boundaries and are not registered here, then you need a Letter of Permission from the parish where you are registered or within the boundaries where you live
Baptisms are at 11:00 am; Select one:
___ English Baptism (second Saturday of the Month);
___Spanish Baptism (third Saturday of the Month)
Godparent Requirements/nformation:
___I understand that the godparent (either one female, or one male, or one of each) must be at least 16 years old and not bound by any canonical penalty legitimately imposed. Godparent(s) cannot be the mother or father of the child
The following documents and Sacramental records from each godparent must be provided:- Baptism Certificate and Confirmation certificate; and
- If godparents are married, they must have a certificate to show proof of sacramental marriage in a Catholic Church

- That the location of the baptism will be either in the main Church or the San Juan Diego Chapel, depending on the number of the number of registrants. ____

Please note that this schedule and location is subject to change. Also, we do not schedule baptisms during the season of lent, between As Wednesday and Good Friday, and on Holy Saturday, unless its through our Religious Education program. If you have questions about these requirements and check list, please feel free to contact our office.
A separate Parish Registration Form is to be completed in the Parish Office with this completed checklist and the documents requested above. The fee of $50 (cash or Check) is to be paid when you come to register. At that time, the date of the baptism will be confirmed.

SPONSORS FOR BAPTISMS
Whenever a person is baptized, as an infant, as a child, or as an adult, there should be at least one person present who will act as sponsor for the one being baptized. The Sponsor, commonly referred to as one’s godmother or godfather, accepts the responsibility of helping the person grown in the Catholic faith. One who acts as a sponsor for an infant or child agrees to help the parents teach their child about the faith and how to live as a practicing Catholic. One who acts as sponsor for an adult agrees to encourage and support the person, pray with and for the person, and offer whatever help, information, or support is needed while the person is preparing to enter the Church and then in living out the rest of his or her life as a practicing Catholic.
For a person to act as a sponsor for Baptism, he or she must:
- Be at least sixteen years old.
- Have already received all the Sacraments of Initiation (Baptism, Confirmation, and Holy Eucharist).
- Be living in a way that demonstrates one’s faith is strong enough to be able to fulfill the responsibilities involved with being a sponsor.
- If married, sponsor must be married in accord with the laws of the Catholic Church.
